2017-18 School Supply List

2017-18 Extended Day Program

Extended Day forms were sent home with students on Friday, August 11th.  The program will begin on Monday, August 14th. 

Policies and Procedures

1. Five Dollars Registration Fee is required for every student (non-refundable)

2. Twenty Five Dollars per week, per child or five dollars per day for drop-ins’ which is receipted through the school’s records. ABSOLUTELY NO CREDIT!!!  **After three reminders or phone calls to pay your fees, your child will be dismissed from the program.**

3. If a school calendar week has less than five days, charges will be five dollars for each day.

(i.e. Labor Day Week)

4. There will be a late charge for pick-up after 5:30 pm. The late charge will be five dollars per child for every 10 minutes you are late. This will need to be paid UPON PICK-UP THAT DAY. You will be given ONLY ONE opportunity to bring in the late fee for the NEXT DAY.  After this, your child will be released from the program.

5. The return of check(s), for reason of insufficient funds, will IMMEDIATELY terminate the privilege of paying by check. The bill must be paid in cash or money order, thereafter.

6. For income tax purposes, the parent must keep up with his/her paid receipts, no receipts will be duplicated at any time. The Tax ID number will be given out if needed in JANUARY ONLY.

7. ALL PAYMENTS must be made to the student’s homeroom teacher the first day of each week.

**Please write your phone number at the top of the check and the student’s name in the memo section.**

8. There will be snacks sold during extended day hours. The prices are

.50 & $1.00.
